Box 1a — Name — Legal Name
Enter your name exactly as it appears on your passport. First name, last name. Do not abbreviate.
Example: Ahmed Waqas Mohammed → exactly as passport shows
Box 1b — Name at Birth (if different)
If your name changed after birth (marriage, legal change), enter birth name here. Otherwise leave blank.
Box 2 — Applicant's Mailing Address
Your complete current mailing address including country. This is where the IRS mails your ITIN letter. Use a reliable address you can receive mail at.
Box 3 — Foreign Address
If your mailing address is in the USA (e.g., a US forwarding service), enter your actual foreign residence address here.
Box 4 — Date of Birth / Country of Birth / Country of Citizenship
Enter exactly as shown on passport. Date format: MM/DD/YYYY. Country of birth = where you were born. Country of citizenship = your passport country.

Box 5 — Visa Information
If you entered the USA on a visa relevant to this application, enter the type and number. If you have never entered the USA and are applying from abroad, leave blank.
Box 6a — Foreign Tax Identification Number
Your national tax ID in your home country (if applicable). Optional but helpful. If your country has a national tax ID, enter it here.
Box 6b — Previous ITIN
If you are renewing an expired ITIN, enter your existing ITIN here. For new applications, leave blank.
Box 6c — IRSN
IRS-assigned number (rare). Leave blank for most applicants.
